The city of Oak Ridge Parks and Recreation Department requires that all teams, coaches, players, and support staff sign their medical release form before being allowed to play in the Oak Ridge Baseball Complex. I know some teams will be scheduled in other locations and might not play any games in the Oak Ridge Complex; however, most of the championship games will be scheduled in that complex. Therefore, we are asking ALL teams to please have this form filled out and send the original to us. Again, this form is strictly for the City of Oak Ridge, and will be the property of Oak Ridge. It will not be viewed unless an incident is reported. I think it also goes without saying; this is not a replacement for your team insurance.

The roster below is to be filled out also. This form will be used by the tournament as your official roster. ALL players that are members of your team, and anticipating participation in the tournament should be listed.

Please completely fill out both forms and mail them to the address below.

I need the original medical release mailed. They will not accept copies.

Please have them to me at least 2 weeks prior to the tournament.
